Billing terms in plain English

Allowed amount
The maximum amount your insurance uses when calculating what the plan and patient may owe for this service.
Contractual adjustment
The part of a charge an in-network provider agrees to write off. It isn't billed to you.
Coinsurance
A percentage of the allowed amount you pay after your deductible is met.
Explanation of Benefits (EOB)
Your plan's summary of how a claim was processed. It isn't a bill, but it says what you should owe.
Good Faith Estimate
A written cost estimate you can receive before scheduled care if you're uninsured or paying yourself.
Itemized bill
A line-by-line list of every charge, with codes and units. It's what makes a real coding review possible.
